1. The filling out and submission of this form is a requirement before I can begin your critique!
2. Fill out all of the required fields and click SUBMIT.  It will be sent to me via an email.
3. Make your payment via the PayPal button on the previous Portfoilo Critique page.
4. Make sure that you have submitted your artwork via the Portfolio Critique Upload process on
the previous Portfolio Critique page.
5.  Once I have all of your artwork, your completed form and payment, I will complete your
critique and get back to you within 2 weeks.  Thank you for allowing me to help you in your
